A special concession applies, allowing FinSide clients and dependants currently active on their medical scheme membership to transfer to KeyHealth without underwriting, waiting periods or late joiner penalties.

Step 2: Complete your Termination Notification
To formally terminate your Sizwe Hosmed membership, you will be required to complete and submit a termination form. Select the option below that applies to the way in which you currently pay your monthly medical scheme contributions.

Termination Letter 1: Select this form below if you currently pay your contributions only at the end of month.

If you're electing to join KeyHealth, the signed and completed termination form can be uploaded via the electronic KeyHealth form in Step 3 below, or emailed to us at medical@finside.co.za.
