Following our recent communication regarding the option available to you to transfer to KeyHealth Medical Scheme, we would like to reassure you that the FinSide team is fully on hand to support you every step of the way.

Chronic conditions: Important information
If you or a dependant have an existing chronic condition, you are required to complete the KeyHealth Chronic Form after you submitted your KeyHealth Transfer Form. This form is used to capture your chronic condition and medication details for KeyHealth as part of the membership process.
Please note that submitting this form does not constitute final registration.

Once your KeyHealth membership has been activated, KeyHealth’s dedicated chronic medicine management partner, MediKredit, will contact you directly to finalise the chronic registration and confirm any additional information or documentation required. Ensuring the form is completed accurately and in full will help avoid delays in this process.
